Stepwise reconstruction of knee stability and physiological kinematics in acute knee dislocation. The operative technique is demonstrated in a case of multiligamentous injury of the knee with involvement of both cruciate ligaments and additional medial and lateral peripheral injuries (type IV according to Schenck classification). Critical soft tissue conditions, infections, old age, obesity, lack of compliance. Time-limited arthroscopy in order to primarily identify and treat posterior horn/root injuries of the meniscus and concomitant intra-articular injuries. Anatomical placement of anterior cruciate ligament (ACL) drill wires for later ACL tunnel drilling is arthroscopically guided. Subsequent conversion to an anteromedial arthrotomy and ligament bracing of the posterior cruciate ligament. The ACL is reconstructed using the ipsilateral semitendinosus tendon. Medial and lateral peripheral injures are anatomically reconstructed followed by a posterolateral augmentation in a technique described by Arciero. Limited weight bearing for 6 weeks and stepwise increase of flexion using a standard knee brace and close clinical monitoring. Ligament bracing of both cruciate ligaments is an established treatment technique in acute knee dislocations and has been proven to achieve good to excellent clinical results. In an ongoing clinical study primary ACL reconstruction as a modified treatment approach indicated superior stability in a 12-month follow-up in patients with acute knee dislocations. OPERATIONSZIEL: Ziel der operativen Versorgung einer akuten Kniegelenkluxation (Schenck IV) ist die Wiederherstellung einer stabilen Bandführung und einer physiologischen Kinematik des Kniegelenkes. Demonstration der Operationstechnik bei einem Patienten mit akuter Kniegelenkluxation (Schenck IV) mit Ruptur beider Kreuzbänder und zusätzlich der medialen und lateralen peripheren Gelenkstabilisatoren. Kritische Weichteilverhältnisse, kritische Gefäßsituation, (Risiko eines intraoperativen Gefäßverschlusses). Beginn mit einer kurzen Arthroskopie zu Identifikation und Adressierung von Meniskusverletzungen und ggf. Platzierung der Zieldrähte für die Ersatzplastik des vorderen Kreuzbandes (VKB). Im Anschluss anteromediale Arthrotomie und „ligament bracing“ des hinteren Kreuzbandes. Das VKB wird mit einer autologen Sehne rekonstruiert. Rekonstruktion der medialen und lateralen Bandkomplexe mit Nahtankersystemen und augmentierende posterolaterale Bandplastik nach Arciero. Teilbelastung mit 20 kg für 6 Wochen. Stufenweise Freigabe der Beweglichkeit mithilfe einer Bewegungsorthese unter engmaschiger klinischer Kontrolle im Sinne der geführten Arthrofibrose. Das Konzept des Ligament bracing des zentralen Pfeilers bei der akuten Kniegelenkluxation wurde bereits wissenschaftlich untersucht und erzielt gute bis sehr gute Ergebnisse. In einer vergleichenden Untersuchung des „VKB internal bracing“ gegenüber der VKB-Ersatzplastik im Rahmen der Akutversorgung einer Kniegelenkluxation zeigt sich ein Trend für überlegene Stabilitätsergebnisse im Rolimeter®-Test (Fa. Aircast, Freiburg, Deutschland) sowie in der Stressradiographie (Fa. Telos
