[Periodontitis - infection or inflammation?]

Periodontitis is a complex, multifactorial disease. Multiple factors such as (epi)genetic factors, environmental factors (microbial biofilm), lifestyle (smoking, stress) and general health (diabetes mellitus) contribute to the development of periodontitis. A healthy subgingival microbiome is in balance with its host, is very stable and diverse, and keeps the host healthy. Changes, such as declining oral hygiene, lead to changes in the microbial composition in the subgingival sulcus: anaerobic and protein-degrading microorganisms with pro-inflammatory properties increase in number and disturb the proportions, leading in turn to changes in the subgingival environment and an increase in pro-inflammatory microorganisms. If the first line of the immune system is unable to restore subgingival equilibrium, microorganisms and their products invade the periodontal soft tissues, resulting in activation of osteoclasts and, ultimately, in the destruction of the periodontium and the onset of periodontitis. Parodontitis is een complexe, multifactoriële ziekte. Meerdere factoren zoals (epi)genetische factoren, omgevingsfactoren (microbiële biofilm), leefstijl (roken, stress) en de algemene gezondheid (diabetes mellitus) dragen bij aan het ontstaan van parodontitis. Een gezond subgingivaal microbioom is in balans met de gastheer, is heel stabiel en divers en houdt de gastheer gezond. Bij veranderingen, zoals verslechterde mondhygiëne, verandert de microbiële samenstelling in de subgingivale sulcus: anaerobe en eiwitafbrekende micro-organismen met ontstekingsbevorderende eigenschappen nemen in aantal toe en verstoren de verhouding, wat vervolgens leidt tot veranderingen in de subgingivale omgeving en een toename van ontstekingsbevorderende micro-organismen. Als de eerste linie van het immuunsysteem niet in staat is het subgingivaal evenwicht te herstellen, dringen micro-organismen en hun producten in het zachte weefsel van het parodontium binnen, wat resulteert in activatie van osteoclasten en uiteindelijk tot afbraak van het parodontium en het ontstaan van parodontitis.
