Résumé

This paper reviews some basic and some new concepts in the diagnosis of mesothelioma. The term "malignant mesothelioma" is no longer recommended; rather, any tumor labeled "mesothelioma" is presumed to be malignant. Clinical and radiologic information is very useful in the diagnosis of mesothelioma; in particular, nodular pleural thickening on CT is usually a marker of malignancy. The literature on markers that separate mesotheliomas from metastatic carcinomas has become very complex and frequently misleading, with many recommended markers actually demonstrating poor specificity. However, newer data show that a combination of HEG1 (clone SKM9-2) and claudin‑4 staining provides extremely high accuracy in separating epithelioid mesotheliomas from non-small-cell lung carcinomas with just two immunostains. This combination works at other sites as well, but caution should be used when high-grade serous carcinoma is in the differential, because all "mesothelioma" markers can also stain high-grade serous carcinomas. There are, unfortunately, no sensitive or specific markers for sarcomatoid mesotheliomas. A variety of immunohistochemical and fluorescence in situ hybridization (FISH) markers are useful in separating benign from malignant mesothelial proliferations; immunohistochemal staining for BAP1, MTAP (or CDKN2A FISH), and NF2/Merlin (or NF2 FISH) will enable the diagnosis of most mesotheliomas. Mesothelioma in situ is now recognized as either a single layer of bland cuboidal mesothelial cells that have lost BAP1, and sometimes MTAP, on immunohistochemical staining, or a process that is morphologically identical to a well-differentiated papillary mesothelial tumor that has lost BAP1/MTAP. Mesothelioma in situ probably always progresses to invasive mesothelioma, but this process is often quite slow. In der vorliegenden Übersichtsarbeit werden einige grundlegende und einige neue Konzepte für die Diagnosestellung eines Mesothelioms erörtert. Die Bezeichnung „malignes Mesotheliom“ wird nicht mehr empfohlen, vielmehr wird jeder Tumor, der als Mesotheliom bezeichnet wird, als maligne angesehen. Für die Diagnosestellung eines Mesothelioms sind klinische und radiologische Informationen sehr nutzbringend, insbesondere stellt eine noduläre Pleuraverdickung in der Computertomographie gewöhnlich einen Hinweis auf Malignität dar. Die Literatur zu Markern für die Unterscheidung zwischen Mesotheliomen und metastasierenden Karzinomen ist sehr umfangreich geworden und häufig in die Irre führend, denn viele empfohlene Marker weisen in Wirklichkeit nur eine geringe Spezifität auf. Jedoch zeigen neuere Daten, dass eine Kombination aus HEG1- (Klon SKM9-2) und Claudin-4-Färbung eine äußerst hohe Genauigkeit für die Unterscheidung epitheloider Mesotheliome von nichtkleinzelligen Lungenkarzinomen mit nur 2 Immunfärbungen bietet. Diese Kombination funktioniert ebenso gut in anderen Bereichen, aber Vorsicht ist geboten, wenn ein hochgradiges seröses Karzinom in der differenzialdiagnostischen Abwägung enthalten ist, weil alle Marker für „Mesotheliome“ auch hochgradige seröse Karzinome färben können. Auch gibt es keine sensitiven oder spezifischen Marker für sarkomatoide Mesotheliome. Eine Reihe von immunhistochemischen und Fluoreszenz-in-situ-Hybridisierungs(FISH)-Markern ist hilfreich bei der Unterscheidung zwischen benignen und malignen Mesothelproliferationen; die immunhistochemische Färbung mit BAP1, MTAP (oder CDKN2A-FISH) und NF2/Merlin (oder NF2-FISH) ermöglicht die Diagnose der meisten Mesotheliome. Ein Mesothelioma in situ wird nun verstanden als entweder eine einzelne Schicht blander kuboidaler Mesothelzellen mit Verlust von BAP1 und manchmal MTAP in der immunhistochemischen Färbung oder als ein Prozess, der morphologisch identisch mit einem gut differenzierten papillären mesothelialen Tumor mit Verlust von BAP1/MTAP ist. Wahrscheinlich entwickelt sich ein Mesothelioma in situ immer zu einem invasiven Mesotheliom, aber dieser Prozess ist oft recht langsam.
