National Consensus on Contraindications for Corneal Donation for Transplantation in Switzerland.

To establish a national consensus on contraindications for corneal donation for transplantation in Switzerland. Swisstransplant (SWT), the Swiss national foundation coordinating tissue and organ donations, convened a working group consisting of six national corneal surgeons and eye bankers and donation experts to create a contraindication list for corneal donation. The group reviewed available national and international guidelines and recommendations, while adhering to Swiss law and transplant regulations. In cases of opposing opinions, the group held follow-up meetings until a consensus was reached. A consensus was defined as agreement among all parties present. From March 2021 to November 2021, the study group held six meetings and created a standardized minimal contraindication list for corneal donation in Switzerland. Thanks to this list, SWT has created a mandatory working and documentation file for donor coordinators to use when evaluating multiorgan donors for corneal harvesting. The authors agreed that while the national consensus list provides standardized minimal contraindication criteria, local eye banks may choose to introduce additional, more rigorous criteria. Given that corneal transplantation is the most commonly performed transplantation, establishing a consensus on contraindications is crucial for recipient safety. The creation of a consensus on contraindications for corneal donation in Switzerland is an essential contribution to fulfil the legal requirements concerning quality assurance and provides sufficient high-quality donor tissue within the country. Therefore, periodic review and revision of the consensus is considered critical. Erarbeitung eines nationalen Konsenses über die Kontraindikationen für Hornhautspenden zur Transplantation in der Schweiz. Swisstransplant (SWT), die nationale Stiftung zur Koordinierung der Gewebe- und Organspende in der Schweiz, berief eine Arbeitsgruppe aus 6 nationalen Hornhautchirurgen, Augenbankern und Spendeexperten ein, um eine Kontraindikationsliste für Hornhautspenden zu erstellen. Die Gruppe überprüfte die verfügbaren nationalen und internationalen Richtlinien und Empfehlungen und hielt sich dabei an das Schweizer Recht und die Transplantationsvorschriften. Bei gegensätzlichen Meinungen hielt die Gruppe Folgesitzungen ab, bis ein Konsens erreicht wurde. Ein Konsens wurde definiert als Übereinstimmung aller anwesenden Parteien. Von März 2021 bis November 2021 hielt die Studiengruppe 6 Sitzungen ab und erstellte eine standardisierte Liste mit minimalen Kontraindikationen für Hornhautspenden in der Schweiz. Dank dieser Liste hat die SWT ein verbindliches Arbeits- und Dokumentationsdossier erstellt, das die Spenderkoordinatoren bei der Beurteilung von Multiorganspendern für die Hornhautentnahme verwenden müssen. Die Autoren waren sich einig, dass die nationale Konsensliste zwar standardisierte minimale Kontraindikationskriterien enthält, dass aber lokale Hornhautbanken zusätzliche, strengere Einschlusskriterien einführen dürfen. In Anbetracht, dass die Hornhauttransplantation die am häufigsten durchgeführte Transplantation darstellt, ist die Schaffung eines Konsenses über Kontraindikationen für die Sicherheit der Empfänger von entscheidender Bedeutung. Der Konsens ist ein wesentlicher Beitrag zur Erfüllung der gesetzlichen Anforderungen an die Qualitätssicherung und zur Bereitstellung einer ausreichenden Anzahl hochwertiger Spendergewebe im Land. Die regelmäßige Überprüfung und Überarbeitung des Konsenses sind entscheidend.
